Overview
The patient uses vitals monitoring tools at home to keep track of blood pressure, blood sugars, ECG, blood-oxygen levels, and other relevant data. The telehomecare part of the system brings that data back to the hospital, where professionals are responsible for tracking the data. 24-hour personalized medical management services are provided to each case to help them take control of intermittent or sudden medical conditions. A variety of personalized medial management services are provided to keep each patient's medical condition under control.

  1. Over 3,400 patients cared for under telehomecare provided by the center.
  2. Blood pressure and blood sugars are significantly controlled or reduced in patients who have participated in the telehomecare program.
  3. A generational study of chronic cardiovascular disease sufferers revealed that of the 53 patients (9.3% of the participats) receiving telehomecare died in the duration of the study, far fewer than the 136 (11.5% of the participants) patients in the control group who died during the course of the study. The hazard ratio in terms of the death rate for telehomecare intervention was 0.866. 
  4. Cases participating in the center's telehomecare system show lower frequency and term of emergency room, hospital, and ICU visits.
Service Procedure

 
  1. Operations for telehomecare: the case manager creates the case file for each patient, compiles the personal data, including diagnoses, medical history, currently used medicines, past hospital stays, cardiac catheterization history, blood reports, and other test results.
  2. Services include: 24-hour, live-updates of vital data/scheduled telephone follow-ups and 24-hour call-in and call-out services/ provides patients with clinic, hospital, and emergency visits/ monthly case and attending physician health reports.
